Could also occur under the following conditions where:

 

The user presses the login button…and because the connection is slow, they
press the login button again…..could this cause, have the effect of their
being 3 records in the connections table with token_status UNUSED?....

When a client (web browser) hands the gateway a token, and the gateway
checks the token status with the central server, and the central server
replies with status DENIED (token already used/user banned/etc...), the
gateway re-directs the user's web browser to that URL.
